"He explains: 'Medical care in the USA is a privilege of the wealthy and the well insured. Children can get care but there's a lot of people who fall through the cracks.

'Around 50m people don't have access to essential services at all – we see people who have a full mouth of rotting teeth and have the worst infections you will see anywhere in the world.'"
